Cash will be rewarded via Visa gift cards and distributed at the end of each month. Contractors must participate in the $50 Off Promo to qualify for the Customer Referral Program. This promotion is only valid for the Gravity Wall Furnace with PilotPro (Models: W255GPPD, W355GPPD, W505GPPD) and does not include the PilotPro Kit (Model PPKNG01). Total dollars rewarded per person across all Cozy rebates cannot exceed $600 per year. Offer good in U.S.A./ Canada only, on Cozy products purchased by any licensed installer from any authorized Cozy Heating Systems LLC wholesaler. In order to qualify for the $50 off promotion, you must submit the online form with all required fields. $50 off promotion can only be redeemed once per customer. Purchase(s) of eligible product(s) made from a participating Cozy dealer or wholesaler must be made between [August 15th, 2019, and March 31st, 2020] to be eligible for the offer. Incomplete or illegible submission forms and claims that do not comply with the terms and conditions of this offer will be rejected and will not be returned.
